Not All Shin Pain Is “Shin Splints”!

The most common cause of exercise-induced lower leg pain is a condition called Medial Tibial Stress Syndrome (MTSS), often incorrectly used interchangeably with “shin splints”.

Understanding the difference is essential to treating your symptoms correctly—and avoiding long-term damage.

What Is Medial Tibial Stress Syndrome?

MTSS is an overuse injury affecting the lower leg, typically triggered by repetitive high-impact activities such as running, jumping, or dancing.

The condition progresses in stages:

  • Stage 1: Microtears occur in the muscles surrounding the tibia (shin bone), causing mild discomfort.
  • Stage 2: Continued stress leads to inflammation and increased tension in the fascia (the connective tissue wrapping around the shin muscles).
  • Stage 3: If left untreated, persistent stress on the shin bone can cause tiny stress fractures—what most people refer to as true “shin splints”.

At this final stage, complete rest is often necessary, and recovery can take several weeks.

Other Causes of Lower Leg Pain

Not all shin pain is caused by MTSS. Less common conditions that can mimic shin splints include:

  • Chronic Exertional Compartment Syndrome
  • Popliteal Artery Entrapment Syndrome

An accurate diagnosis from a podiatrist is essential to rule these out.

Signs & Symptoms of MTSS

MTSS usually starts as a dull ache along the inner edge of your shin (medial tibia) after exercise. If left unaddressed, it can develop into sharp pain during activity and even at rest.

You may also notice:

  • Pain worsens with running or jumping
  • Tenderness when pressing along the shin
  • Possible lumps or bumps along the shin (localized inflammation)
  • Tightness in the lower leg muscles
What Causes Shin Splints?

Shin splints often occur when someone ramps up training too quickly—“too much, too soon”. This leads to overload on the muscles, bones, and connective tissues of the lower leg.

Contributing factors include:

  • Overpronation (feet that roll inwards), increasing stress on shin muscles
  • High-arched, rigid feet that reduce shock absorption
  • Tight calf muscles or restricted ankle mobility
  • Weak glutes and hips, reducing lower limb control
  • Inappropriate footwear—unsupportive or worn-out
  • Running on hard surfaces like concrete or bitumen
  • Poor running technique or gait mechanics

The right treatment depends on the severity and cause of your symptoms. Early intervention often leads to faster and more complete recovery.

Treatment options may include:

  • Targeted calf and shin stretches
  • Trigger-point therapy and foam rolling
  • Massage and dry needling/acupuncture
  • Gradual modification of training load (e.g. swapping running for swimming or cycling)
  • Strengthening exercises for glutes and hips
  • Footwear recommendations or upgrades for better support and cushioning
  • Custom foot orthotics to address biomechanical issues
  • Gait retraining to improve running efficiency
